At the bottom of this page are Biological Evidence Kit forms.
Serology: This sub-discipline performs examinations of evidence for the purpose of identifying biological substances. Of interest are blood and body fluids which may proceed to DNA analysis.
DNA: This sub-discipline performs examinations of biological substances for identification based on accepted forensic testing methods.
CODIS, Combined DNA Index System: CODIS is made up of databases which can be searched against each other. While the most common use of the system is to search unsolved crime scene evidence against known convicted offenders, the system can be used to match up missing persons and unidentified human remains. Each CODIS participating state is networked to the FBI, which allows all states to search at a national level.
